Dietitian

An expert in dietetics; that is, human nutrition and the regulation of diet. A dietitian alters their patient’s nutrition based on their medical condition and individual needs. Dietitians are regulated healthcare professionals licensed to assess, diagnose, and treat nutritional problems.

Dietitian services offered are to both adults and adolescents and at an out of pocket cost.

FeNO Testing

FeNo stands for fractional exhaled nitric oxide. When you breathe out, your breath can show if you have inflamed airways. A FeNO test measures the amount of nitric oxide in your breath. This test helps Providers tell if and how much inflammation you have in your airways.

FeNO levels help Providers determine if your current management plan is working properly and how your medicines need to be increased or reduced, can track your airway inflammation over time and will tell your Provider if you have been taking your medication properly. Also, FeNO levels help your Provider know how well you may respond to corticosteroids and can rule out other conditions that have similar symptoms.

Our office is the only office in the area offering this test.

Although some insurance companies cover this test, others consider it an experimental test and do not cover the cost. This tool is very valuable as it decreases the chances of obtaining or treating the wrong condition by 95%. We offer the test at a cash rate of $34.00.

Injections

If a patient as injectable medications and/or vitamins that have been prescribed by one of our Providers, we will perform the injection for the patient.

We also provide certain injectable medications for our patients, including but not limited to Depo-Medrol and Rocephin. These medications must be ordered by one of our Providers.

Consultations

Understanding our Chronic Care Management Program is important to us. Therefore we provide patients and their families a FREE 15-minute consult before signing up for the program and annually to answer any questions.

Notary Services

A notary is a person licensed by the government to perform such acts in legal affairs, in particular, witnessing signatures on documents.

Notary services are offered for a fee of $10.00 per two (2) pages.

Advanced Care Planning

Dedicated to helping patients talk about their wishes for end-of-life care is important to us. We believe that the place for this to begin is at the kitchen table – not in the intensive care unit with the people we love before it is too late. Together we make these difficult conversations easier. We can make sure that your wishes and those of our loved ones are expressed and respected.

You may obtain an Advance Care Planning (ACP) packet from your Provider or the Chronic Care Management Team. You may schedule this visit with your provider by calling 772-283-4428 Ext. 4.

Annual Wellness Visits

Annual Wellness Visits are yearly appointments to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors. Keep in mind that the Annual Wellness Visit is not a head-to-toe physical.

To properly document your Annual Wellness Visit, we are required to check your height, weight, blood pressure, and other routine measurements. We are required to give you a health risk assessment, review your functional ability and level of safety, learn about your medical and family history, make a list of your current providers, durable medical equipment supplies, and medications. We must create a written five (5) to ten (10) year screening schedule or check-list, screen for cognitive impairment, including diseases such as Alzheimer’s and other forms of dementia, screen for depression, and provide health advice and referrals to heal education and/or preventive counseling services aimed at reducing identified risk factors and promoting wellness.

Due to the extensive information collected, these services are provided by a certified clinician before you see your provider. The results are shared with your provider to review and provide feedback as they deem necessary. Annual Wellness Visits are covered in full, by Traditional Medicare. If you have a Medicare Replacement Plan, we urge you to contact your insurance to inquire about your benefits.

Insurance Planning Assistance

Insurance plans can be very confusing as more and more make their debut we want to ensure that our patients are cared for in the manner in which they can afford. If you find yourself uncertain about what plan to choose you may schedule a phone appointment with our Practice Administrator, and she will walk you through the process, so you are completely informed of the choice benefits thus allowing you to make an informed decision.

In-Office Procedures

Our in-office procedures include EKG’s, overnight pulse oximetry, six (6) minute walk, peak flow, and blood sugar testing. All procedures are ordered by your healthcare professional. Ask your healthcare professional if you should have any questions.

Chronic Care Management

Chronic Care Management (CCM) is a non-face-to-face service provided to patients who have multiple chronic conditions. The conditions are expected to last at least 12 months and normally require medication to maintain. The Centers for Medicare and Medicaid recognizes that CCM services are a critical component of health care that reduces overall health care costs. Although this program is primarily a Medicare-funded program commercial payors do recognize the value for their beneficiaries.

Remote Patient Care Monitoring

In its simplest form, Remote Patient Care Monitoring (RPM) involves the use of connected electronic tools that record personal health and medical data in one location. The data is then reviewed by a provider when there is an abnormal trend. Parameters goals are set for each patient when they are enrolled. Health systems are utilizing RPM services to treat both acute and chronic conditions and are not limited to clinical improvements alone. The care modality can also help break down hurdles related to social determinants of health, that is, social factors that negatively affect health.

Pulmonary Function Tests

Pulmonary Function Testing (PFT) is a noninvasive test that shows how well the lungs are working. The test measures lung volume, capacity, and rates of flow. The primary purpose of the PFT is to identify the severity of pulmonary impairment utilizing the measures mentioned above. We have our own PFT machine and a Respiratory Therapist that comes one to two weeks per month to perform our PFT testing.
