Our Telehealth platform is now active.

Telehealth, also known as telemedicine, is a virtual visit similar to FaceTime and Skype.

‍Please call our office 772-283-4428 Ext. 4 to schedule your Telehealth visit.

‍If your visit is not scheduled, our Physicians/Physician Extenders will not be available to you.

If you were directed to our Telehealth platform, please click on the below Physician/Physician Extender you were scheduled with or spoke with for the upcoming visit.

If you want to request an unscheduled visit, please select our Physician/Physician Extender on call button to request your visit.

To participate in Telehealth, you must have one of the following

Dr. Charnvitayapong has been in private practice for more than 20 years. He specializes in Pulmonary Diseases and practices Pulmonary and Critical Care Medicine.

Dr. Weakley
Internal Medicine

Dr. Weakley has been in private practice for more than 20 years. She is an Internist, specializing in Adult Primary Care.

Dara is a Board-Certified Nurse Practitioner and a current member of the American Association of Nurse Practitioners.

Kushboo Patel
APRN

Kushboo Patel is a Nurse Practitioner for our Pulmonary Physicians. She has extensive experience with diagnosing and treating Pulmonary Disease in an outpatient setting.

Rebecca Beyer
APRN

Rebecca Beyer is a Nurse Practitioner with her own Internal Medicine Practice. She has extensive clinical experience as a Nurse Practitioner.

Shweta Frechette
APRN

Shweta is a Nurse Practitioner for all our Physicians. She has experience with diagnosing and treating Pulmonary Diseases.

Leslie Clifton
PA-C

Leslie is the Physician Assistant for Dr. Dermarkarian. She has experience with diagnosing and treating Pulmonary and Infectious Diseases.

Appointment Cancellation Policy

PIMA requires a minimum of 24 hours notice to cancel a scheduled appointment. Failure to provide us with at least 24 hours of notice of cancellation prior to your appointment will result in:

  • A $35 fee for Office Visits
  • A $50 fee for Pulmonary Function Test

PIMA now has the availability to allow patients to keep their credit card on file for their balances with us. If this is something you wish to do please print the form below and bring it in with you during your appointment. We will be happy to set everything up for you at that time.
